Administrative Forms:
MYSA Official Match Lineup
This form is mandatory for all games. You are required to present to the referee prior to every game. The game cannot begin without this form....do yourself a favor and make multiple copies for your coaches folder.
MYSA Medical Release
This form is mandatory to carry with you at all times. It is required by all sanctioned tournaments and should be used to reference emergency contact information for the players on your team.
Ejected Participant Report
Hopefully you never need this....but, be prepared. If a player is ejected (receives a red-card) you must have the referee fill this form out after the game.
Financial & Miscellaneous Forms:
Tournament Check Request
This will be used if your team will register for a tournament, turns in money to our treasurer and wants the club to process a check or electronic payment.
Club Reimbursement Request
If you incurred expenses that were pre-approved by the NLS administration, please complete this form for reimbursement.
Team Deposit Request
Use this form to request a deposit to your Northern Lights Soccer Club team account for such things as tournament registration, apparel or other team events.
